What Is a Surgical Extraction?

A surgical extraction is a procedure used when a tooth cannot be easily accessed or removed in one piece. Unlike a simple extraction — where the tooth is loosened and lifted out — a surgical extraction may involve:

  • A small incision in the gum tissue to expose the tooth
  • Removal of a small amount of bone tissue surrounding the tooth
  • Sectioning the tooth into smaller pieces for easier removal

The procedure is performed under local anaesthetic to ensure you feel no pain. Sedation is also available for patients who feel anxious or nervous about the treatment.

When Is a Surgical Extraction Needed?

Your dentist may recommend a surgical extraction in a number of situations where a standard extraction would not be sufficient:

  • Impacted wisdom teeth — fully or partially trapped beneath the jawbone or gum tissue
  • Teeth broken off at or below the gumline, leaving no visible crown to grip
  • Teeth with curved, long, or unusually shaped roots that resist straightforward removal
  • Teeth that haven't erupted properly and are causing pain or crowding
  • Failed previous extraction attempts where the tooth or roots remain

We'll always carry out a thorough assessment — including X-rays — before recommending surgical extraction, and we'll explain your options clearly so you can make an informed decision.

The Procedure

Assessment and imaging — detailed X-rays are taken to plan the safest, most effective approach.

Anaesthesia — the area is fully numbed with local anaesthetic, with sedation available if preferred.

Incision — a small, precise cut is made in the gum to access the tooth and surrounding bone.

Removal — the tooth is carefully removed, sometimes in sections, to minimise disruption to surrounding tissue.

Closure — dissolvable stitches are placed if needed, and full aftercare advice is given before you leave.

Recovery After a Surgical Extraction

Most patients recover well after a surgical extraction. Here's what to expect in the days following your procedure:

  • Swelling and mild discomfort for a few days is normal — this typically peaks around day two and then subsides
  • Pain relief medication and antibiotics may be prescribed to manage discomfort and prevent infection
  • Soft foods are recommended for the first few days — avoid anything hard, crunchy, or very hot
  • Avoid smoking, alcohol, and vigorous rinsing for 24–48 hours to protect the healing site
  • Most patients recover fully within one to two weeks

NHS

Surgical extractions are available on the NHS when clinically necessary — for example, impacted wisdom teeth causing pain, infection, or damage to adjacent teeth. Treatment is provided at standard NHS Band 2 charges.

Private

Private surgical extractions offer faster access, more flexible appointment times, and sedation options for anxious patients. Ideal if you want to be seen quickly or prefer a more relaxed treatment experience.
